Release of Liability

On behalf of myself, my heirs, executors, agents, assigns, and representatives, hereby indemnify, release and forever hold harmless Synergy Dance Company, LLC, its directors, employees, instructors, volunteers, and facilities used, from any and all claims of liability arising from any accident, personal injury, death, or property loss or damage sustained by my child/myself/the minor child for whom I am a legal guardian, while that person is participating in activities connected with Synergy Dance Company, LLC, including, but not limited to, classes, rehearsals, performances, competitions, or other activities.

Assumption of Risk

I understand that dance activities have inherent risks of injury, and, being fully aware of all risks, I consent to have my child/myself/the minor child for whom I am a legal guardian, participate in the programs and activities offered by Synergy Dance Company, LLC, and I accept full responsibility for providing adequate health and accident insurance coverage for the protection of my child/myself/the minor child for whom I am a legal guardian, who participate in these programs/activities.  By signing this statement, I declare that the participant is in good health, with no physical conditions that might prevent his/her/my participation in strenuous and rigorous dance activities and other training and performances.
Further, I understand and acknowledge that, because of the physical nature of dance, there may be physical contact between directors, employees, staff, company members, instructors and students during rehearsals, shows, workshops, productions, and especially during dance instruction.  I understand that, at times for proper instruction and safety, physical contact is necessary.

Media Release

I authorize Synergy Dance Company, LLC, to use photos and videos of my child/myself/the minor child for whom I am a legal guardian for promotional purposes.  I understand that, in my capacity as the legal guardian of a minor, I am authorizing Synergy Dance Company, LLC to use photos and videos for promotional purposes.

Enrollment Policy

All classes are subject to a minimum and maximum enrollment.  Synergy Dance Company, LLC reserves the right to change dates and times of classes and/or adjust class ages to meet either minimum or maximum enrollment restrictions.   For a group class not meeting minimum enrollment, you will have the option of paying duet/trio class pricing.

Payment Policy

All student accounts will be set up with auto deduction and fees will be charged on the 1st of each month.
Costume fees will be added to the November deduction and are eligible for refund up to December 18th.
